The Office of Community & Population Health (OCPH) at Montefiore designs, implements and support interventions that improve the health and well-being of our community, which includes our patients, our associates and the residents of the neighborhoods they live in. We focus on the prevention of chronic disease, and therefore the priority initiatives are aimed at improving nutrition, increasing physical exercise, reducing smoking, increasing connectivity to care and promoting healthy parenting. Through our Healthy Store Initiative (MHSI), Montefiore’s OCPH is working to increase community access to healthy foods and beverages by partnering with local bodegas and community based organizations to create sustainable food retail change.
